Account & Client Setup

All ordering physicians must provide complete information, including the NPI # and/or UPIN #, the first time a specimen is submitted. Once that physician is added to our database, the physician’s name, office location and Genex LPC Account # are sufficient. Therefore prior to sending your first specimen to GeneX LPC you must submit an account/client setup form. Account/client setup forms can be downloaded from the buttons above. Our staff are here 24/7 to help you set up your account and respond to your questions.

Clients may choose to pay their invoice online. A direct account must be set up prior to submitting specimens for testing. If a specimen is received without an established account, results will be held until an account is established. To set up an account with GeneX LPC fill out the Account Setup Form and email, mail or fax it to:
